You have to first get the "Good" ending, by getting everyone's CG cutscenes before the end of the game.

You can easily do this by saving and starting a new game AFTER Sayori confesses to you and BEFORE she dies.

Just put the "script" file from the download inside the games "game" folder, overriding the existing "script" file.
